Mount Madonna School portraits Thursday, September 19th and Friday, September 20th at Upper Campus under the big oak tree. Portraits will be taken at the start of school, and all orders are due in the office on Picture Day.

• Please complete order form on the reverse side and staple payment (check or cash) to the order form. Then return the order to the office or your child’s teacher by the day of pictures. Please make checks payable to “Michele Gunsky”.

• If you have more than one child attending Mount Madonna please complete one order form for each child, but you may write one check. Staple all orders forms and payment together before turning in.

• Additional order forms can be found on the school website or in the front office
